Patient Rights
As a patient at Zreik Medicine, you have the right to:
- Dignified, respectful, and non-discriminatory care regardless of age, gender, race, national origin, religion, sexual orientation, gender identity, disability, or source of payment.
- Privacy and confidentiality when speaking to a healthcare provider and in the handling of your medical records.
- All communications and records pertaining to patient care shall be treated as confidential in accordance with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and other applicable federal and state laws.
- Patients have the right to review and obtain a copy of their medical records and to request amendments to their records in accordance with HIPAA and applicable state law.
- Information about your diagnosis, treatment plan, and prognosis so you can make informed decisions about your health.
- Participate in your care.
- Know your physician’s qualifications.
Patient Responsibilities
As a patient, you have the responsibility to:
- Provide accurate information about your symptoms, lifestyle choices, and medical history for accurate diagnosis and safe treatment.
- Report unexpected changes in your condition to the responsible practitioner
- Participate in your care by asking questions about your condition or seeking clarification.
- Follow the provided treatment plan and, where needed, discuss potential issues with your primary health physician to address emerging concerns.
- Provide accurate billing and insurance information, and pay for the healthcare services provided.
- Inform your providers about any advance directives and provide copies of such documents for inclusion in your medical records.
- Keep appointments and, when unable to do so for any reason, notify the Practice in advance.
- Inform your providers about any obstacles you anticipate in complying with prescribed treatments.
- Accept the consequences of your actions if you refuse treatment or do not follow the practitioner’s instructions.
- Provide information concerning your ability to pay for services and to work with the Practice to make payment arrangements when necessary.
- Be considerate of the rights of other patients and Practice personnel.
